Social Worker Position

Salary: $55,000 - $65,000

Position Overview:

Cooper Square Committee is hiring a licensed social worker to serve primarily low income seniors residing in a multi-building cooperative with over 400 residents (Cooper Square MHA II HDFC). Approximately 30% of the residents are seniors. Our goal is to help senior residents to age in place as long as possible with a strong network of case management and wellness programs. Many of the seniors face chronic and complex conditions (e.g., chronic medical and mental health diagnoses, limited personal resources). Targeted Case Management services will be tailored around a patient’s life goals and designed to provide high quality care.

• The social worker will engage in assessments and provide case management, counseling for senior residents for the treatment of mental illnesses and other challenges, including grief, trauma, loneliness, social isolation and relationship problems with family members.
• The social worker will interface with Resident Relations Specialists, Benefits Specialists and Visiting Nurse staff to jointly identify residents who are in need of various types of case management or interventions to ensure they are accessing financial and housing resources available to them in addition to clinical resources.

Day-to-day duties of the social worker will include:

• Help residents maintain optimum health or improve functional capability.
• Engage in assessments, create a case management and treatment plan including referrals.
• Maintain resident case files, including income and medical records and documentation consistent with state regulations.
• Participate in continuing education as required by NY State.
• Other related duties as deemed necessary.

Qualifications:

• Master's degree in Social Work (MSW) and/or Licensed Clinical Social Worker (LCSW)
• Knowledge of community resources and counseling / social work practices with high-risk populations
• Experience working with diverse, low income people including seniors and residents who have chronic medical conditions and/or mental health challenges
• Experience working within housing developments, NORCs or as part of neighborhood preservation efforts is a plus

• Candidates who are bi-lingual/fluent Spanish speakers are strongly preferred

Schedule/Pay Rate:  

• Full-time (Monday to Friday, 9 am – 5 pm)
• Pay Rate: $55,000 - $65,000 depending on experience
